Conditions：

1. Drivers are above 18 years old and hold driver licenses. Guardians on behalf of adolescents who are below 18 years old sign the contract and get an approval letter issued by FASC;

2. Drivers from Hong Kong, Macau, Taiwan, and overseas must have valid identify cards and driver licenses.

Submission Regulations：

1.Must fill in items with ‘*’

2.Filled name and ID number must be valid, true and the size of submitted photo must be below 800 KB
